By submitting this form, you are notifying Le Jardin Academy that your child is absent. It will serve as an excuse note for the absence. For late arrival, early pick-up, or multiple day absences, please fill out the NOTE section at the end of this form.

*Please call the office if you are submitting an early release request for the current day. 

This form MUST be filled out by a PARENT/GUARDIAN using their email only. Students are not permitted to fill out this form.
An email receipt of the form will be sent immediately to the parent/guardian. The school office will verify that the email address used on the form belongs to the parent/guardian.
