Summer Fun Activities

Health Requirements

Grades K-12 Health Requirements:

Link to NEW State of Hawaii Health Requirements
Current TB clearance and physical by a U.S. doctor.
Student Health Form 14 to prove that a physical examination was performed; or
A signed statement or appointment slip from your doctor to prove that your child has a physical examination scheduled and/or has begun the vaccination series and is waiting for the next dose in the series.
